FMF Qualification Packages

The following documentation is necessary to submit in order to be considered for the official FMF Board.

Initial Qualification Package

Initial Qualifications means that you are currently not FMF qualified.

• Personnel Qualification Standard (CORE)
• Personnel Qualification Standard (Platform – LCE / GCE / ACE)
• Performance Evaluation Report (Last EVAL)
• PRIMS Report (BOL)

Re-qualification Package

Re-qualification means you already have an FMF qualification but you need to re-qualify in a different platform.

• Performance Evaluation Report (Last EVAL)
• PRIMS Report
• Previous FMF Qualification Certificate or Page 13 from previous qualification
